Job Description

SUMMARY 

Under general supervision of the Director, the Quality Improvement Specialist (QIS) provides quality improvement (QI) support to a defined number of nursing homes (NHs) throughout the state.  QI support consists of conducting on-site visits, teleconferences, Webinars and Learning and Action Network (LAN) meetings with Health Services Advisory Group (HSAG) participating NHs.  The QIS will establish and maintain strong relationships with NHs throughout the project; provide QI consultation to NHs. 

Responsible for coordinating improvement activities for the assigned nursing homes engaged in the Clostridium Difficile Infection (CDI) Initiative. This includes performing risk assessment and gap analysis; develop action plans and priority lists; recommend best-practice tools and resources; conduct on-site and virtual visits, and coaching calls; and assist nursing homes in entering monthly CDI data in the Centers for Disease Control and Prevention (CDC) National Health Care Safety Network (NHSN) to reduce/eliminate Health Care Associated Infections (HAIs) in the nursing home setting. 

Topics include healthcare acquired infections/conditions, falls, pressure ulcers, readmissions, sepsis, adverse drug events and antibiotic stewardship, and TeamSTEPPS®. Also responsible for coordinating project activities including planning, report preparation and finalization, meeting project timelines and deliverables. 

ESSENTIAL COMPETENCIES, DUTIES, AND RESPONSIBILITIES

  • Serves as a primary HSAG contact with assigned NHs and communicates/consults with them on a regular basis to assist with their interventions related to established Centers for Medicare & Medicaid Services (CMS) goals, objectives, and measures, and to provide technical assistance and recommendations as needed.

 

  • Provide technical assistance and troubleshooting for the Center for Disease Control (CDC) National Healthcare Safety Network (NHSN) tracking system for Healthcare Associated Infections (HAI)

 

  • Communicates/consults with nursing home-based infection prevention teams to identify gaps and implement action plans to reduce/eliminate HAI in the nursing home setting.

 

  • Communicates/consults with nursing home providers on a regular basis to assist with advancing their quality improvement objectives, and to provide technical assistance and recommendations as needed.

 

  • Provides necessary education and materials and directs providers to resources that will assist them with understanding and utilizing problem solving and quality improvement techniques.

 

  • Assists in the development of training programs and the provision of technical assistance to providers, partners, stakeholders and community members and participates in educational presentations as required.

 

  • Assesses current NHs’ processes, support systems, and barriers related to the CMS Quality Assurance & Performance Improvement (QAPI) focus. Based upon assessment findings and in collaboration with the NH team, determines appropriate interventions and modifications, and shares these with the appropriate NH staff.

 

  • Able to communicate and work well on tasks with stakeholders to plan, facilitate, and execute interventions.

 

  • Utilizes clinical expertise, problem-solving techniques, flow diagrams, statistical process control charts, Advancing Excellence Campaign tools and resources, QAPI and other methods to demonstrate to NHs how to reengineer existing processes or design new ones to improve performance.

 

  • Routinely documents and reports project status, on-site visits, provider interventions, activities, and barriers/solutions, using appropriate HSAG and/or CMS reporting tools and databases.

 

Job Requirements



EDUCATION AND/OR EXPERIENCE 

  • Candidate must be an experienced healthcare professional with a minimum of a Bachelor's degree in appropriate science or related discipline. 
  • Must have a background in a NH setting and/or clinical background. 
  • Three or more years of healthcare experience in a hospital and/or NH. 

 

OTHER QUALIFICATIONS 

  • Quality management experience and knowledge of QAPI tools and techniques. 
  • Experience specific to NH management is preferred. 
  • Clinical experience specific to infection control, antipsychotic medication reduction, pressure ulcer reduction, and/or physical restraint management is preferred. 
  • Possess general understanding of the changing healthcare environment. 
  • Proficient in Microsoft Office systems. 
  • Proficient in oral and written communication and interpersonal skills.
